Classify findings:
- User-visible (must not leak diagnostics)
- Ops-visible (structured logs, trace events)
- Dev-only (move to scripts / gated)

Convert into a PR ladder:
- One concern per PR: logging, error handling, config defaults, MCP bootstrap, etc.
